Your phone may ask for your PUK code after you've entered your PIN incorrectly too many times. This is a theft prevention device, so there's no way to retrieve it yourself.

If you call us, we will provide you with your PUK. Please enter it carefully - if you enter it incorrectly too many times, we'll need to send you a new SIM card.

Once you've entered your PUK successfully, it will ask you to create a new PIN. Afterwards, you will be able to use the phone as usual.
